What is a hexagonal connection in dental implants, and how does it work?

The hexagonal connection is the mechanical interface between the dental implant body and the prosthetic abutment. The hexagonal geometry (six sides at 120°) performs a fundamental anti-rotational function: it prevents the abutment from rotating around the implant axis under masticatory loads. In the internal hexagon variant, the hexagonal socket is machined into the interior of the implant body, providing a larger contact surface and better force distribution compared to the classic external hexagon.

What is the difference between dental implants with an internal hex and those with an external hex?

In implants with an external hexagon (the original Brånemark system), the anti-rotational geometry protrudes from the coronal plane of the implant: the abutment rests on top of it. This design has the advantage of a long clinical history, but it features a reduced contact surface and a greater tendency for the prosthetic screw to loosen under oblique loads. In implants with an internal hexagon, the hexagonal socket is machined deeper into the implant body: the connection is more stable, the lever arm on the prosthetic screws is reduced, and the risk of micromovements at the interface is significantly minimized. This translates into greater prosthetic longevity and a reduction in mechanical complications over time.

How does the hexagonal prosthesis work on implants and abutment teeth?

The hexagonal prosthesis on implants and abutment teeth is a hybrid solution in which the prosthesis is supported simultaneously by osseointegrated implants (with a hexagonal connection) and by natural teeth prepared as abutments. The implant’s hexagonal index ensures precise and repeatable positioning of the prosthetic abutment, guaranteeing proper transmission of masticatory forces. What is meant by “single prosthetic platform” in hexagonal implants?

“Single prosthetic platform” means that the hexagonal connection size is identical for all diameters within the same implant system: an abutment designed for the Touareg™-S with a diameter of Ø 3.75 mm is therefore also compatible with diameters of Ø 4.2 mm and Ø 5.0 mm. This approach significantly simplifies the management of prosthetic component inventory, reduces selection errors, and lowers stocking costs for both the dental practice and the dental laboratory. In combination with integrated platform switching, it also helps preserve marginal bone tissue.
